Your First Appointment
Please click on the Patient Forms link above, download, fill out, sign and then email the forms before your initial examination

Please arrive 15 minutes early so that we can complete your initial paperwork. Also, please bring the following
- Patient’s insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
Ghadeer Okayli, M.D. is a provider for Seton Health Plan. She provides insurance billing for this health plan only. You are responsible for co-payments or charges that are not covered by your insurance. All other client are seen on a pay for service basis. She will provide you with the necessary paperwork to file for reimbursement by your health care insurance. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Privacy Practices
Our Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can get access to this information. It is given to all new patients and a copy is available upon request.

Online evaluations
Dr. Okayli is now accepting online evaluations through skype for patients who wish to visit her from the comfort of their chosen location. All you need is a computer with a camera attached to it. This route is for established patients only. All new patients need to be seen face to face at our office location for the initial visit. Please note that Seton Health Plan DOES NOT pay for online evaluations. If you are a Seton covered patient and wish to do that you will be responsible for the full payment of the session.
Mobile Office
A fully equipped mobile office allows for onsite( at a mutually agreed upon location) or home visits. Dr. Okayli will park at your driveway or at a mutually agreed upon location and invite you into the mobile office for the evaluations. The Mobile office is not marked with a logo or advertisement graphics for the purpose of maintaining confidentiality.
